- Practicum & Student Teacher Verification Form
- Call (434) 939-9000 to speak with Lori Shifflett, Central Office Receptionist, regarding fingerprinting, background check, and $37 processing fee (cash or check only).
- TB screening (communicable tuberculosis) results. The screening or skin test must have been completed within the last 12 months.
